Forms for marriage, baptism, and parish registration are available from our site in .pdf format. These .pdf forms can be downloaded, read and printed using the free Adobe® Acrobat® Reader. Although electronic submission of completed forms is not yet available, you can download, fill out and mail in any of these forms, saving a trip to the office.

The web page application form is in HTML format, as part of a page on the parish site that outlines how to get your group's page up and running. The .htm form can be copied and pasted into a word processor, or printed directly from your browser.

Download and install the free Adobe® Acrobat® Reader if you don't yet have it.

Parish Registration:

Request for Marriage Preparation:

Contact the parish office at 540-659-1102 for more information on marriage preparation.

Baptism Information Form:

Baptism Information sheet: baptisminfo.pdf required please download and complete.

Requirements:

• Parents are to be registered at St. William of York Parish.
• Married in the Church and practicing the Faith.
• Attended a Baptism Preparation class at St. William of York Parish (Sacrament of Baptism)

In addition to fulfilling the above requirements:

• ATTACH sponsor forms for each godparent (obtained from their respective parishes).
• Submit sheet one week prior to the baptism or sooner.

Please note that if any of the paperwork is incomplete, the baptism may be delayed.
